that was most certainly a double charged reload. no amount of fouling would cause this at worst fouling interrupts cycle of operation. whole lot of inexperienced expert shooters. if it was the incorect caliber the bolt would almost certainly not go into battery.

not cleaned not holding it right go to a shooting range and learn how to fire and hold a gun correctly
yep, squib round, bullet lodged in the barrel then fired another one behind it. Blew in the chamber, gun performed as it should have, exhausting all the gasses down into the magazine. Better to lose a finger or two than to lose your head. My bet, cheap ammo.
running higher grain shots than the lesser quality AF can handle, from the looks of the tape on it I'd have to suspect its not well maintained as well
