I knew a guy when I was in high school that had tourettes where he would have involuntary facial tics and one of those pics was winking. I remember we are standing and waiting for the jrotc uniform inspection when one of the seniors walk by and see this guy winking at him. he didn't know this guy had tourettes and starts asking him wth dude. the senior apologized after it was explained and then had a good laugh with us. I think I laugh harder when the senior told the dude I thought you were coming onto me and tourettes guy says dude you are not my type.

My interpretation from experience with family: He appears to have a twitch along with mental health issues, there’s a long pause after he says he has a twitch. During that time the voice(s) say something funny in regard to the stop winking at me comment. Whereupon his response is to call the voice in his head a weirdo, and then laugh because he found the voices comment humorous.
