I'm in pain just looking at him. That has to be so painful. You wouldn't be able to lay down or anything unless you was laying on your stomach. Laying on your side would probably even hurt. That looks like a 24/7 pain. I don't know how they do it. Here in Indiana the emergency room has to see you and treat you even if you don't have insurance. I'm not sure about surgery and stuff like that but they can clean it real good and give you antibiotics and put bandages on it and even send you home with extra bandages. If it's not healing from the antibiotics go back and they might can give you something else. If you don't have insurance you might have to pay for the prescription but hell it's better then dieing.
