That's a real thing in some southeast Asian countries. I worked at a Thai restaurant, and nearly all of the people who came in and could not speak English well enough to order would request fish with the head still on. The restaurant was 15 minutes from the ocean, so the owner always kept fresh fish with the head still on, because at least once a day someone would order a fish head.
