Those aren’t mooring lines, they are dock lines. Stern lines to be more precise and of coarse they fail under full throttle. Now if you are in the bite of that line and it fails it can kill you or take you legs or arms off instantly.

Mooring lines didn't fail, the captains crew failed.
They usually an officer that makes sure all the lines are off. also when the ship comes in makes sure all lines are on. usually the people on the dock release the mooting lines, because guys on the ship can't release the lines and swim to catch up with the ship! blame the oicoslj. Thats the officer in charge of shitty little jobs!
