It’s pretty simple if u know how revolvers work it use to happen a lot back in the day but it’s cus the bullet in the rotating cylinder is not aligned perfect with the barrel Ehat it need to be to shoot properly
my step son bought a used handgun, there was round stuck in the barrel. he's lucky I took home to clean it up for him or this would have happened to him.

didn't put the cylinder all the way In so the bullet didn't line with the barrel if you notice the cylinder didn't turn even tho he pulled the hammer all the way back that means the cylinder wasn't all the way in and it didn't line the round and didn't go down the barrel it stayed in position was ignited bY the hammer hitting the primer and boom it exploded very simple if u watch the video and actually know about guns 😂

That must be a Dan Wesson not a Smith & Wesson 686 l frame because mine never did that and I had it for 4 years. I heard that Dan Wesson to do that often
