The guy in the water was frozen in fear. Everything was going slow motion for him nearing the end. The friend, with only pebbles to throw at the slow creeping reaper, decided that his sharp elbow is his best weapon and thrust it in like a spear and inflicted a piercing blow to the reaper's snout stunning it. The savior friend did the right thing. Otherwise, the trauma seeing his friend die the way it would have been will surely consume him to death.
