People who are seriously overweight are not necessarily overeating. Some have painful digestive disorders. Others have to take medication, which causes the person to gain weight, such as steroids. Some can't afford healthy foods or have no access to them. Some just don't know how to cook and eat what they were fed as a child. They eat small amounts of food, but it's high carbs and fillers. They suffer with extreme pain and infections. Once you get to a certain size, your body isn't strong enough to exercise. You need help to shower and use the restroom because you can't reach. You can't breath, or clean. So it gets worse, which makes you worse. It's sad and a disability & often not their fault.
